TO REGISTER:

1. Download the above forms

2. By July 1st 2010 - TO REGISTER AND HOLD A SPOT - (spots fill up quickly!! First Come First Serve!)
Send in:

• Registration Form
• 1st deposit of $900 in a check for made out to Fuel Lacrosse

3. By July 20th 2010 - Send in:

• Transportation Form
• Medical form – 2 pages
• Waiver – 2 pages
• Photo form
• Remainder of the balance - $1600 in a check for made out to Fuel Lacrosse

4. Check email and www.fuellacrosse.com for confirmation materials to print out.

5. An email will be sent out with travel information and where to meet
